Operated Internationally by the West Midlands Ambulance Service NHS Trust, this annual subscription service is available with your Medi-Tag product. In addition to information engraved on your Medi-Tag, you have more details held on a secure computer database – you can update these details as many times as necessary.

If needed, this option allows medical professionals to access additional medical details
24 hours a day, 365 days a year.
Your Medi-Tag is engraved with:
  • Your Name
• 24 Hour Emergency Telephone Service number
   (prefixed with the UK international dialling code)
• Your unique membership number – used to access your  personal/medical data
• Vital medical information about your condition
As well as FREE engraving, the membership joining fee (currently £10.00) and first years’ annual subscription charge (currently £20.00) are FREE with your first product. Subsequent annual subscription payments are requested by post in the month before the Membership Renewal date.
IMPORTANT:

• On becoming a member you will enter into a Contract with the NHS Trust. The Contract must be read and agreed by the person to be registered, or  parent/guardian if under 16.
• The W.M.A.S. NHS Trust issue a hard copy of the database Membership Register. For security purposes, this is sent to the name and address registered for membership.

If you cannot fit all of your details in the space provided, please ask your Doctor’s advice on your most vital information to be known in an emergency.
