How to order a Medi-Tag falls into the following categories:-

Ordering a Product & Service Option

A Service Option is only available with a Medi-Tag product.

Each Medi-Tag product purchased must be personalised by choosing one of two Service Options

There are a number of methods for ordering a Medi-Tag Product and Service Option:-

Online


  1. Select your Product
    To find a product click onto Product Selector to show the product categories in a drop down menu, click on the product category to view the products. You will be presented with thumbnail images of all products in that category. Select the product you are interested in by clicking on the image. You are then presented with an enlarged image of the product with the price(s). If you wish to browse the complete product category, click the previous and next buttons on the large thumbnail picture. Once you have found the product you are interested in, click the ‘Select and Personalise’ button.
  2. Personalise your Product
    You are presented with the choice of two Service Options for personalising your product. Choose by clicking the ‘Select Option 1’ or ‘Select Option 2’ button:-

    Personalising your product and Membership Registration falls into two categories:–
    • New Members – if you wish to become a member for the first time
    • Existing or Previous Members – if you are or have been a member of this Service. You must input your Membership Number and Surname (as shown on your Register). We validate the status of your membership and you are automatically taken to the relevant Form. The status will be:-
    - Existing member: Renewal Subscription not due
    - Existing Member: £20.00 Renewal Subscription due
    - Previous Member: £30.00 Re-registration fee to be paid

    To complete the Form we need the engraving you require, your full name, address, telephone number and date of birth. The Form also has optional input fields for details of your medical condition and treatment, your GP and hospital care, and person to contact in the event of an emergency. Once completed, click ‘submit’.

    You are then presented with the engraving you have input and asked to check and verify. Once verified, you are asked whether you wish to save the Option 1 details you have input for personalising another Medi-Tag product (this will include the engraving and Registration Form). You must select ‘yes’ or ‘no’ before proceeding with your order.
    Note: If you select ‘yes’, you should use the saved details to personalise the next product you order.

    The terms and conditions of your Contract with the West Midlands Ambulance Service NHS Trust are then shown to you. You must complete a checkbox to confirm you have read and agree the terms and conditions of the Contract and click ‘submit’.
    Note: This must be agreed by the person being registered, or parent/guardian if under 16.
    You are then presented with a summary of your personalised product before you ‘add’ your order to your shopping basket.

    Select Option 2
    Personalising your product can follow our suggested layout or you can choose your own adaptation and click ‘submit’.

    You are then presented with the engraving you have input and asked to check and verify. Once verified, you are shown a summary of your personalised product before you ‘add’ your order to your shopping basket.

You can either Contact Us for a full colour brochure to be sent to you for your completion, or you can download and print the brochure or ordering forms in pdf format. Then send the completed forms to us with your payment.

Note: For Option 1, Members or parent/guardian if under 16 must sign and date the Registration Form.

Telephone and Fax

You can place an order for Option 2 only - see Contact Us. We will need the product code, engraving requirements, delivery details, the total payment due and your payment details.

Update Register

This is only applicable to Existing Members.

Online

Go to Membership Updates and Update Register Details. Input your Membership Number and Surname (as shown on your Register) so we can validate the status of your membership.

Existing Members then complete the secure Email Form that is sent directly to the West Midlands Ambulance Service NHS Trust. To complete the Email Form you must give your full name, address, telephone number and date of birth – as shown currently on your Register, and your email address. The Form also asks for your updated details. Once completed, click ‘submit’.

Note: Members or parent/guardian if under 16 must complete the email form.

Post

You send your updated details to the West Midlands Ambulance Service NHS Trust.

Note: Members or parent/guardian if under 16 must sign and date the updates.

Renew Subscription

This is only applicable to Existing Members who have received a request to renew their Membership Subscription.

Online

Go to Membership Updates and Renew Subscription. Input your Membership Number and Surname (as shown on your Register) so we can validate the status of your membership.

You are then taken to a Renewal Form to complete and submit (and pay your £20.00 Renewal Subscription). To complete the Form we need your full name, address, telephone number, date of birth and email address. Once completed, click ‘submit’ to add your Renewal Subscription to your shopping basket.

Post, Telephone and Fax

See Contact Us. We will need your Membership Number, the Renewal Date (see top right hand corner of your Register) and your payment details.

Re-register

This is only applicable to Previous Members whose membership has lapsed and do not wish to purchase a new Medi-Tag.

Note: If you wish to Re-register AND order a new Medi-Tag – go to
Ordering a Product and Service Option.

Online

Go to Membership Updates and Renew Subscription. Input your Membership Number and Surname (as shown on your Register) so we can validate the status of your membership.

You are then asked to ‘click here’ to re-register and are taken to a Re-registration Form to re-register your membership and pay your £30.00 fee. To complete the Form we need your full name, address, telephone number and date of birth. The Form also has optional input fields for details of your medical condition and treatment, your GP and hospital care, and person to contact in the event of an emergency. Once completed, click ‘submit’.

After submitting the completed Form you are shown the terms and conditions of your Contract with the West Midlands Ambulance Service NHS Trust. You must complete a checkbox to confirm you have read and agree the terms and conditions of the Contract and click ‘submit’ to add your Membership Re-registration to your shopping basket.

Note: The terms and conditions of the Contract must be agreed by the person being registered, or parent/guardian if under 16.

Online Order Completion


  1. Shopping Basket
    All orders go through to the Hoopers shopping basket. If you want to remove an item use the red minus button in the quantity field. Alternatively, you may delete all items by clicking on the 'clear basket contents' text link. The green plus button in the quantity field is disabled for Medi-Tag items only.

    To complete the ordering process and pay for your goods, click on ‘proceed to checkout’ text link.

    Conversely, you can click ‘continue shopping’ - this will take you back to the home page of the brand that you are in, or alternatively use the logos on the left hand side to go to another brand.

    If you have tried to purchase an item and your shopping basket is showing empty - you will need to change your security settings to enable/accept cookies. Cookies are used to remember the product items in your basket and if you choose, will also allow you to store information to save time completing forms on future purchases.
  2. How to use the Checkout
    Once at the Checkout, please follow the on screen instructions.

    You will need to enter the Delivery Details if you have ordered a product to be delivered. Please complete all mandatory fields shown with a red asterix, it also helps if you have your postcode/zipcode.

    If you have purchased a Medi-Tag product AND another Hoopers brand product, you will be given the choice to have the goods delivered in two deliveries by using the tick box. This will incur two shipping charges.

    Confirmation of your order will be displayed, tick the box if the payment card details are the same as the delivery address to save re typing. Check the delivery address and order details, and if correct click on the 'Pay' button to enter your payment details.
  3. Online Payment
    Paying for your goods is made simple through a secure payment server. All the major debit and credit cards can be cleared, eg. Visa, MasterCard, Delta, Solo and Switch. The service is fully secure, bank endorsed, and utilises a range of Industry Standards combined with fault tolerant secure server networks and firewalls.

    On entering the page you will be asked to enter your Payment Card details, including card number, expiry date and cardholders name. Please ensure that your email address is typed correctly and we have a daytime contact number as this enables us to contact you quickly should there be a query with your order, then click the ‘Make Payment’ button to give permission to complete the payment transaction.
  4. Order Confirmation
    Once payment has been authorised you will be given a reference number - please quote this in all correspondence. An email will be sent confirming your order details.
